Dr. Mario 64 features many different modes of play (all of which have tutorials hosted by a Para-Goom). The player uses the n64 to toggle with the different modes they wish to select. The first one is Classic mode, where Dr. Mario is playable like he was originally. In the second mode of play, Story mode, Dr. Mario or Wario must track down the stolen Megavitamins. This mode only has versus matches against computers. The third mode allows practice against a computer player, where any character found in Story mode can be selected. The fourth game mode is Flash mode, where only the clearing of flashing Viruses counts. The fifth game mode is Marathon mode, which is basically an endless mode (the Viruses never stop coming out of the bottom of the jar). The sixth game mode is Score Attack mode, where there are only three minutes to get both a high score and clear all of the viruses. Multiplayer mode supports up to four players, which adds a mode allowing four players to play on two teams (although teams are locked to P1/P2 vs P3/P4).

In Dr. Mario 64, the Megavitamins belong to Dr. Mario and are contained in a bottle, who used them to cure patients all day long due to them being struck by flu season. Whenever the effects of the Megavitamins are applied, a star appears and envelop the one who consumed them. After both Wario and Mad Scienstein separately witnessed Dr. Mario using his Megavitamins to cure an ailing Octo, they each planned to steal it for their own use: Wario plans to use the medicine to take advantage of the situation to enrich himself, while Mad Scienstein was tasked by a hidden figure to steal them for him. After Mad Scienstein stolen the medicine at Dr. Mario's house when he was not around, Wario discovered the thief and chased him while Dr. Mario returned. Dr. Mario believed Wario to have stolen it in the ensuing chaos until he is proven to not be the thief, but when the real thief scrambled from the scene, both Dr. Mario and Wario chased Mad Scienstein until they reached Rudy's castle. If Dr. Mario was the chosen character, he eventually used his Megavitamins to cure Rudy, who had a cold, but if Wario was the chosen character, Mad Scienstein will snatch the Megavitamins from Wario in the end, prompting a never-ending chase.

The Megavitamins are also shown to have a transforming property when consumed, which is seen in Story Mode after beating 8 opponents without continuing in Normal difficult or higher. In Dr. Mario's case, it causes him to transforms into Metal Mario, whereas in Wario's case, it causes him to transform into Vampire Wario. The effect is implied to be temporary as after the respective characters were beaten, they returned to normal.

Dr. Mario 64 plays the same as its predecessors for the most part. Dr. Mario throws vitamin capsules into a jar full of viruses, and the goal is to line up a minimum of four of a color combination of viruses and vitamins to make them disappear. There are three types of Viruses - the Red Virus (Fever), the Blue Virus (Chill) and the Yellow Virus (Weird), with a vitamin color designed to defeat them.

Dr. Mario 64 features many different modes of play (all of which have tutorials hosted by a Para-Goom). The player uses the +Control Pad to toggle with the different modes they wish to select.

If the player battles against a CPU player, their difficulty is dependent on the selected character, which is indicated by the amount of stars under them. To increase the difficulty, hold L Button and press A Button to increase the difficulty level by one, or B Button to increase the difficulty level by two.
