The level begins with [[Mario]]/[[Luigi]] sliding down a slope that leads into a cave. The player then slides into and kills (unless they jump before hitting them) 4 [[Buzzy Beetle]]s. The slope then drops the player into a small pool of water with 5 [[coin|coins]] over it. Immediately after this is an upward slope which leads to two [[Piranha Plant]]s and a Buzzy Beetle. There are then two different paths; A top path, and a bottom path, both of which lead out of the cave and up to the surface of the cave, where there are 3 large slopes and multiple Green [[Koopa Troopa]]s. The top path is rather short, quickly leading out of the underground section. However, it also has a hidden [[Note Block|Pink Note Block]] in it that takes the player to [[Coin Heaven]]. The bottom path is much longer than the top path, having a large pool of water with 5 more coins in it followed by two small pockets of water and a smaller pool of  water containing a [[? Block]] with a [[Mushroom]]/[[Fire Flower]] inside it, before also leading up to the surface (albeit a bit farther down than the top path). After going forward some more, there is another tunnel that leads to a small area with multiple pipes, two of which have a Piranha Plant and a [[Venus Fire Trap]], respectively. The last pipe is a [[Warp Pipe]] that leads to the end of the level.

Progressing, the player will find two hanging and grounded [[Warp Pipe]]s throughout the level with [[Piranha Plant]]s. After the first pipe, the cave splits into two paths leading either above ground or continuing through the cave. The path leading above ground has a hidden pink [[Note Block]] that will send the player to [[Coin Heaven]], and the path continuing underground has a [[? Block]] with a power-up in it. Both paths will lead to a hill the player has to jump over to reach another cave entrance with more pipes and Piranha Plants, with one at the end of the slope having a [[Fire Piranha Plant|Venus Fire Trap]] in it in the NES version. Going up through the Warp Pipe at the end will take the player to the [[Goal (Super Mario Bros. 3)|goal]] of the level.

In the original NES version, the cave has a white and light blue pallet, though later remakes show it as more of a dirt cave.
