In the past, the Pi'illo used to live happily on this island. But when Antasma tried to take the Dark Stone, everything changed as when the pi'illos were trapping Antasma in the Dream world, he broke the stone. The Nightmare fragments touched each Pi'illo, turning them into stone, and trapping their souls in the dream world.

Peach, Mario and the Toads are welcomed by Broque Monsieur and other bloc people. But when, on the road to castle, they unleash monsters on Peach and the toads, it seems to be a trap. After Mario defeats them, Baroque Monsieur says that it was just another welcome test. At the Pi'illo castle, they are reunited with Luigi, and they encounter Starlow, who is on holidays on the island. She decides to follow Mario, like in Bowser's Inside story.
Soon after, while watching a short documentary about the island's past, Peach and Toads worth are accidentally taken to the underground of the castle. Mario and Luigi follow them and, and they succeed in saving the two. They also find a strange pillow made out of stone. Getting out of this dark place, they find themselves in the collection room of the castle. Luigi asks Broque monsieur if he could sleep on the Pi'illo prince's bed, using the stone pillow. He is allowed to, but when he falls asleep, a portal opens and Peach is sucked inside. Mario jumps in it, and gets in a strange place between the real world and the Dream World. He sees Peach being caught by a dark smoke, and follows her into the dream world.

As he enters it, a light asks him for help. Mario proceeds in the dream, soon joined by Dreamy Luigi, and sees the princess going under the ground, drawn by the dark smoke. It is the home of an ancient pillow-like race known as the Pi'illos and is where Mario, Luigi, Peach, the Toads and Starlow are invited to go on vacation. As with Beanbean Kingdom, all of the areas are named after the theme of the area, except they are all puns on terms of sleep instead of types of laughter. The island's name itself is a homophone of "pillow".

To prevent this, the Pi'illos battled him and managed to seal him in the Dream World, but before he was sealed Antasma shattered the stolen Dark Stone; the broken pieces became Nightmare Chunks and fell all over the island, petrified every single Pi'illo, turning their bodies into stone, and trapped their souls in these Chunks located in the Dream World.
